Charmony Foundation is a 501(c)(3) nonprofit working at the intersection of health and community. We train Community Health Workers, partner with local organizations, and build simple technology that keeps people connected to the care they need.

Our work is grounded in privacy, consent, and plain language. We believe everyone deserves a real person in their corner when navigating health and social services.

Charmony + CAPPA

Charmony Foundation works in partnership with CAPPA, the Community Action Partnership Program Alliance. Charmony trains and supports the Community Health Workers who sit with people one-on-one. CAPPA brings the network of local pantries, housing groups, counseling centers, and veteran services that turn a plan into real help.

Together we run one front door. You tell your story once. From there, your Community Health Worker walks with you and sends referrals to the partners who can help — and follows up until the loop is closed.

What this platform is

This is the tool our team uses to keep track of that work. It holds your intake, your care plan, and the referrals sent on your behalf, so nothing gets lost and you never have to repeat yourself.

About the AI helper

We use AI to help our staff write notes faster and to answer simple questions in the chat window. AI never decides who gets help, what services you receive, or anything about your care. A real person makes every decision. The chat is not medical advice — if you need medical care, call your doctor, and in an emergency call 911.

Our privacy promise

This is the same promise you agree to during intake, written out in full:

  • We keep your information private.
  • We only collect what we need to help you.
  • We only share it with partners who help you, and only with your OK.
  • Partners see your first name and last initial only. They do not see your health details.
  • We keep a record of who looks at your information.
  • You can change your mind at any time. Tell your Community Health Worker and we will stop sharing.
  • A real person — not a computer — makes the decisions about your care.
  • You can ask us what we have about you, and ask us to correct or delete it.
